Role of an intensive health outreach team in improving care outcomes
This article examines the development and function of an intensive health outreach team for people with learning disabilities in Gloucestershire. It analyses health inequalities and their determinants among people with learning disabilities, and applies the Health Equalities Framework to describe the benefits and outcomes of the outreach team’s services.
