Nurse struck off for false entries on patient's fluid intake chart

A care home nurse has been struck off after deliberately falsifying a patient’s fluid intake chart to mislead inspectors during an inspection of the care home. Catherine Smith altered records to suggest the patient consumed liquids, despite none being taken. Ms Smith admitted to the changes, claiming work pressure, but the Nursing and Midwifery Council fitness to practise panel found no evidence supporting her claims and concluded her actions posed a risk to patient safety.
