Patient death: nurses did not understand bed rail assessment rules

Nurses at a care home where a patient receiving end of life care fell out of bed and sustained an injury that contributed to her physical and mental decline did not know the appropriate approach to assess the need for bed rails, according to a coroner who warned that future deaths could occur unless action was taken and ordered the operator to provide details of such action and a timetable for carrying it out.
