Nurses’ confusion over DNACPR contributed to care home death
A coroner found staff at Aspray House Nursing Home in Leyton, London, delayed and mishandled emergency care when Madeline Reding choked and went into cardiac arrest. Despite four nurses being present, cardiopulmonary resuscitation was not started promptly and 999 was not immediately called. Poor leadership, misunderstanding of care directives, and ineffective first aid contributed to her death, the coroner’s prevention of future death report finds.
